A case of 31 yr old male with CLD and PORTAL HYPERTENSION. Hiii, I am A.Roopa Sri,3rd semester student.  This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems w8ith collective current best evidence based inputs. Hi, I am Roopa Sri, 3rd semester student.This is an online elog book to discuss our patients health data after taking his consent.This also reflects my patient centered online learning. portfolio.                                                                                      55 yr old patient came to OPD with complaints of weakness right upperlimb and lowerlimb , slurring of speech, fever,chest pain.                                                           History of present illness : patient was apparently asymptomatic 15days back, he developed pain at right upper limb and lower limb . Investigation showed  K+ -2.15 mg/dl, hb-8.8gmpercent . History of past illness : No similar complaints in the past  Complaints of asthma, tuberculosis ,epilepsy, diabetes mellitus since 10yrs  Family history : Father - blood cancer leukemia  Mother-diabetic No history of past surgeries  No history of pedal edema Sleep: 12 am to 7am Bladder :5 to 6 times per day Bowel: regular
